Hubungan antara Diameter Atrium Kiri dan Kejadian Fibrilasi Atrium pada Pasien Penyakit Jantung Reumatik

Abstrak
Fibrilasi atrium merupakan salah satu komplikasi penyakit jantung reumatik yang paling sering terjadi. Tujuan penelitian ini adalah mengetahui hubungan antara usia dan diameter atrium kiri dengan kejadian fibrilasi atrium pada pasien penyakit jantung reumatik. Metode penelitian ini adalah observasional analitik dengan desain potong lintang. Data diambil dari rekam medis pasien penyakit jantung reumatik di RSUP Dr. Hasan Sadikin selama tahun 2016. Metode pengambilan sampel yang digunakan adalah consecutive sampling. Data dianalisis menggunakan uji chi-square. Dari 161 pasien penyakit jantung reumatik, 123 pasien dipilih sebagai subjek penelitian. Hasil penelitian menunjukkan mayoritas berusia >44 tahun (40,7%) dan perempuan (69,9%). Pasien yang memiliki fibrilasi atrium sebanyak 113 orang (91,9%). Sebanyak 118 pasien (95,93%) memiliki lesi katup kombinasi dengan kelainan katup terbanyak adalah mitral stenosis, yaitu 111 orang (90,24%%) diikuti regurgitasi trikuspid 100 orang (81,3%) dan regurgitasi aorta 81 orang (65,8%). Sebanyak 56 pasien mengalami dilatasi atrium kiri berat dengan fibrilasi atrium (96,6%). Uji chi-square menunjukkan hubungan antara diameter atrium kiri dan kejadian fibrilasi atrium bermakna. Terdapat hubungan yang bermakna antara usia dan kejadian fibrilasi atrium pada pasien penyakit jantung reumatik (p<0,05). Simpulan, terdapat hubungan bermakna antara usia dan diameter atrium kiri dengan kejadian fibrilasi atrium pada pasien penyakit jantung reumatik.

Abstract
Atrial fibrillation is one of the most common complications of rheumatic heart disease. The purpose of this study is to determine the relationship between age and left atrial dimensions with the incidence of atrial fibrillation in patients with rheumatic heart disease. The method of this research is observational analytic with cross sectional design. Data taken from medical records of patients with rheumatic heart disease in Dr. Hasan Sadikin General Hospital Bandung during 2016. The sampling method used is consecutive sampling. Data were analyzed using chi square test. Patients diagnosed with rheumatic heart disease counted 161 patients, 123 patients were selected as study subjects. The results showed that the majority patients were > 44 years (40.7%) and female (69.9%). Patients who had atrial fibrillation were 113 (91.9%). A total of 118 patients (95.93%) had a multiple valve lesion with the most common lesion is mitral stenosis (90.24 %%), followed by tricuspid regurgitation in 100 patients (81.3%) and aortic regurgitation in 81 patients (65.8%). A total of 56 patients with severe left atrial dilatation suffering from atrial fibrillation (96.6%). The chi-square test shows the relationship between left atrial dimensions and incidence of atrial fibrillation in patients with rheumatic heart disease. There was a significant association between age and incidence of atrial fibrillation in patients with rheumatic heart disease (p<0.05). In conclusion, there was a significant association between age and left atrial dimensions with the incidence of atrial fibrillation in patients with rheumatic heart disease.
